Our full‑time Social Services professional supports elders and their families from the first inquiry through every step of the stay.
Support Includes
- Guidance through admission and care transitions
- Advocacy for elders throughout their stay
- Collaboration with hospital discharge planners and referring facilities
- Assistance planning a safe return home or transition to a lower level of care
- Counseling and participation in the Care Plan Team
A Partner for the Whole Family
Moving into a care community, or transitioning between levels of care, can be overwhelming. Our Social Services team keeps families informed at every step, coordinates regular Care Plan Team meetings, and is always available to answer questions or talk through concerns.
We can also help connect elders and families with community resources in Lincoln and Lancaster County, such as Aging Partners and local support groups, so no one has to navigate the process alone.
